Counseling Session Process

To start the counseling session, the Registered Dietitian would serve you with more accuracy if she has access to your
  1.     Medical history
  2.     Latest blood works results
  3.     Diet history, habits and eating behaviors
  4.     Three-day food record
  5.     List of taken medication, supplements, or natural health products
  6.     List of your physical activity( type of activity,  intensity, and frequency)
  7.     Glucometer readings ( self-monitoring blood glucose results if you have diabetes)

To individualize your nutrition care plan, the Registered Dietitian would spend more time at the initial appointment to complete the comprehensive assessment and review all information and anthropometric data( height, weight, waist circumference, and body fat percentage) which affects your health and nutrition status. At the end of the initial assessment, she assists you to set achievable and realistic client-centered goals that tailor your need. A nutrition care plan would be set according to your goals that include an individually-designed meal plan, physical activity regimen, list of triggering motivation items, and some practices for behavior change model to warrant your continuous and permanent lifestyle modification. The follow-up sessions will be set to monitor your progress on a weekly or bi-weekly manner.

The Registered Dietitian provides a continuous support for email or phone call between your two appointments. Therefore, she would be accessible when you have a question or concern or any hesitation about your plan.

The Counseling service including initial assessment and follow-up sessions are fee-for-service based. Please inform your dietitian at the first session, if you have an extended health care plan. To warrant your coverage for a dietetic service, contact your insurance company and ensure that you have adequate coverage for this service.
